Clearance Process

Boys Team - Please complete by June 30, 2023

Players may not participate in any practice's or games until they have been cleared.

After completing the online portion, print the confirmation message and signature pages.

Bring both pages to both locations below to be signed.

Stop #1: Student Store -

Get required signature and turn in participation donation of $540 check payable to PVHS ASB The student store is open from 8am-noon, Monday-Friday for the remainder of June 2023. It is closed this year during the month of July.

Stop #2: Athletic Office -

Turn in the confirmation message and signature page along with a hard copy of your physical. The Athletic Office is open daily from 7:30am-3:30pm. They close for lunch and short breaks daily.
